THIS FORM UPDATES A CLIENT'S INCOME, NET WORTH, INVESTABLE ASSETS, INVESTMENT PRODUCT KNOWLEDGE, AND ASSETS HELD OUTSIDE THE FIRM. IT IS AN INTERNAL FORM USED BY THE FIRM, AND REQUIRES NO SIGNATURE FROM THE CLIENT. I MISTAKENLY PUT INFORMATION IN THE WRONG LOCATION. TO CORRECT MY OWN ERROR, I USED WHITE OUT TO CORRECT THE MISTAKE AND INPUTTED THE INFORMATION IN THE CORRECT LOCATION. AFTER THE FORM WAS CORRECTED, I SIGNED THE DOCUMENTS AND SUBMITTED THE FORM. I WAS NEVER PROVIDED WITH THE POLICY FROM THE FIRM THAT STATES AN INTERNAL FORM CANNOT BE CORRECTED BY THE REGISTERED REPRESENTATIVE. I RESIGNED PRIOR TO ANY FINAL CONCLUSION IN ORDER TO PURSUE OTHER OPPORTUNITIES.
