THE FIRM FOUND I DEPOSITED A CHECK INTO THE CLIENT'S IRA ACCOUNT WITHOUT FIRST CLARIFYING THE CLIENT'S DEPOSIT INSTRUCTIONS. AS A RESULT, THE CLIENT PROCESSED AN EXCESS REMOVAL CAUSING HER TO INCUR TAX PENALTIES. THE FIRM OFFERED REIMBURSEMENT FOR THE TAX PENALTY ($41.39) IN ADDITION TO THE SALES LOAD INCURRED ($154.30) WHEN THE FUNDS WERE INVESTED IN THE CLIENT'S IRA. THE CLIENT DECLINED THE FIRM'S OFFER.
