Broker Comment: THE IR ADVISED THAT HE RECEIVED INCORRECT INFORMATION FROM THE INSURANCE COMPANY WHEN IR CONTACTED INSURANCE COMPANY AS TO THE COST BASIS OF THE ANNUITY AND HOW MUCH COULD BE WITHDRAWN WITHOUT ANY TAX LIABILITY OR PENALTY. IR STATED THAT BASED ON THE INFORMATION PROVIDED BY THE INSURANCE COMPANY, IR WITHDREW $15,000 AT THE REQUEST OF THE CLIENT. AS A RESULT OF THIS TRANSACTION, THE CLIENT INCURRED ADDITIONAL TAXES OF $6,225 ON THE DISTRIBUTION. THE FIRM REIMBURSED THE CLIENT THE AMOUNT OF THE ADDITIONAL TAXES INCURRED.
