The 401K company did not withhold state taxes which I was not aware of the time. I was a stay at home mom and this was my only income. I was unable to pay for quite some time and finally filed CH 7 bankruptcy which was discharged in May of 2015. I thought that this was included in the discharge and I have not seen it on my credit report since then. I haven't received any documentation from PA Dept. of Revenue all week to set up something to satisfy the lien. A deferred payment plan agreement was issue on 10/27/2016 with the amount of $110.00 initial payment and payments of 106.00 on the 15th of every month to be satisfied by 11/15/2019.
