Broker Comment: IN NOVEMBER OF 1978, MY WIFE AND I WERE DIVORCED. I MOVED AWAY BUT SHORTLY MOVED BACK HOME AFTER SHE AND I DECIDED TO TRY TO PUT THINGS BACK TOGETHER AGAIN. I RETURNED HOME WITH EVERY INTENTION OF MAKING THIS RELATIONSHIP WORK. BUT DID NOT ACTUALLY RE-MARRY MY FORMER SPOUSE. WE LIVED TOGETHER FOR ALMOST SEVEN YEARS AND HAD A THIRD CHILD. MOST OF THIS TIME, MY FORMER WIFE DID NOT WORK OR HAVE ANY OTHER MEANS OF SUPPORT OTHER THAN MYSELF. I HAD NOT NOTIFIED THE BANK OF ANY CHANGE IN MY MARITAL STATUS OR CHANGE IN MY MEDICAL DEPENDENT COVERAGE AT THIS TIME, AND HAD CONTINUED TO PAY BY PAYROLL DEDUCTIONS FOR FAMILY PLAN COVERAGE. SEVERAL YEARS AFTER MY DAUGHTER WAS BORN AND THINGS WERE STILL PROGRESSING, MY FORMER SPOUSE NEEDED FOOT SURGERY. THIS TOO, ALONG WITH THE BIRTH OF MY DAUGHTER, WAS PAID IN PART THROUGH MY EMPLOYER. FINALLY, IN JUNE OF 1986, WE BOTH AGREED THAT OUR RELATIONSHIP WAS NOT GOING TO WORK SO WE SEPARATED AGAIN. NO CLAIMS FOR ANY DEPENDENT OTHER THAN MY CHILDREN WERE PUT THROUGH AFTER THIS TIME. I WAS MARRIED IN MAY OF 1988 AND I WENT TO THE PERSONNEL DEPARTMENT TO HAVE A NEW WIFE'S NAME ADDED AS MY SPOUSE, AS WE ARE EXPECTING A CHILD LATER THIS YEAR. MY FORMER EMPLOYER SAID THAT MY PREVIOUS SPOUSE STILL SHOWED ON MY PERSONNEL RECORDS AS MY DEPENDENT. I SAID I WAS SORRY BUT HAD NEGLECTED TO HAVE HER REMOVED. AFTER FURTHER CHECKING THEY INFORMED ME THAT THEY HAD PAID OUT SEVERAL CLAIMS, NAMELY THE BIRTH OF MY DAUGHTER AND HER FOOT SURGERY. I SAID I HAD MADE A MISTAKE IN NOT CHANGING THE INSURANCE AND ASKED THAT THE BANK LET ME KNOW WHAT AMOUNT OF MONEY WAS INVOLVED, AND I WOULD REPAY IT IN FULL. HOWEVER, ACCORDING TO PERSONNEL POLICY, THIS WAS NOT ACCEPTABLE AND AS A RESULT, I LOST MY JOB.
