Broker Comment: CLIENT WAS ADVISED OF THE RISKS OF INVESTING. THE PAST HISTORY OF MARKET RETURNS WAS DISCUSSED, AND NO ASSURANCES WERE GIVEN THAT THEY COULD BE ATTAINED. CLIENT ANSWERED QUESTIONS ON HIS INVESTMENT EXPERIENCE, AND AN INVESTMENT PROFILE WAS FILLED OUT DETAILING HIS INVESTMENT OBJECTIVES AND RISK PROFILE. FROM THE CLIENT?S RESPONSES TO THE INVESTMENT QUESTIONNAIRE PROFESSIONAL MANAGERS WERE DISCUSSED AND ONE WAS SELECTED. THE ACCESS PROGRAM AGREEMENT AUTHORIZES THE ACCESS MANAGER TO BUY, SELL, OR OTHERWISE TRADE SECURITIES OR OTHER INVESTMENTS WITHOUT DISCUSSING THESE TRANSACTIONS WITH THE CLIENT OR UBS|PAINEWEBBER. NO COMMISSIONS WERE CHARGED FOR ANY TRADES SINCE THE ACCOUNT WAS MANAGED ON A FEE BASIS. THE CLIENT WAS SENT PERFORMANCE REVIEWS ON THE MANAGER ON A QUARTERLY BASIS SINCE THE INCEPTION OF THE ACCOUNT. ON APRIL 16, 2001 WE MET WITH THE CLIENT, HIS ACCOUNTANT AND REV JAMES MAYWORM. AT THAT MEETING PERFORMANCE REVIEWS WERE DISCUSSED WITH THE CLIENT AND HIS ACCOUNTANT. IT WAS DECIDED AT THAT MEETING TO CONTINUE WITH THE ACCESS PROGRAM.
