Improperly witnessed the type written application; submitted an application he knew was incomplete or improper to Horace Mann; and information spouse and customer that they would have adequate life insurance coverage if they executed the Horace Mann life insurance policy. Damage Amount Requested: $480,000.00 Broker Comment: Mr. Brown was shown an application which he then signed. The allegations are unwarranted and unsubstantiated.
