THE CLIENT STATES THAT THE IR SHOWED HIM OTHER CUSTOMERS ACCOUNTS WHICH HAD DONE WELL AND ASSURED CUSTOMER THAT HIS INVESTMENT WOULD GROW OR DOUBLE. THE CLIENT IS NOW ASKING THAT HIS ACCOUNTS BE REVIEWED BY THE FIRM BECUASE IN HIS OPINION, BASED ON HIS AGES, INCOME AND INVESTMENT KNOWLEDGE, THESE INVESTMENTS WERE INAPPROPRIATE. AMOUNT MAY EXCEED $5,000.

Broker Comment: THIS ANNUITY WAS PURCHASED OVER EIGHTEEN MONTHS AGO, AND THE CLIENT NEVER EXPRESSED ANY DISSATISFACTION WITH THIS INVESTMENT. THE CLIENT SIGNED THE HARTFORD APPLICATION, WHEN THESE FUNDS WERE ROLLED OVER FROM HIS VERIZON COMPANY PLAN. THE APPLICATION INDICATED THE FUNDS SELECTED AND PERCENTAGES THAT HE WANTED TO INVEST INTO EACH SUB ACCOUNT. THE CLIENT RECEIVED THE CONTRACT AND THE PROSPECTUSES ASSOCIATED WITH THE ANNUITY AND THE FUNDS. THE IR STATED THAT HE NEVER OFFERED ANY GUARANTEES OR ASSURANCES ON THE PERFORMANCE OF THIS INVESTMENT. THE REQUEST FOR REIMBURSEMENT WAS DENIED.

THE CUSTOMER ALLEGED THAT IF HE WERE PRESENTED WITH THE AN ACCURATE PRESNTATION OF THIS INVESMENT, HE NEVER WOULD HAVE MADE THE PURCHASE. THE ANNUITY HAS DECLINED IN VALUE IN EXCESS OF $5,000.

Broker Comment: THE IR INDICATED THAT ALL OF THE FEATURES OF THIS ANNUITY WERE PROPERLY DISCLOSED, AND THAT HE NEVER INFERRED THAT THERE WAS ANY GUARANTEE THAT THE PRINCIPAL COULD/WOULD BE RETURNED IN TWO YEARS. THE IR PROVIDED A PROSPECTUS TO THE CLIENT AT THE TIME THE INVESTMENT WAS SOLD AND DELIVERED THE CONTRACT. A COPY OF THE SIGNED RECEIPT DATED 5/22/01 WAS SENT IN AND IS PART OF THE FILE. THE CLIENT'S REQUEST FOR REIMBURSEMENT WAS DENIED.
